The NCSSM CARENetwork Report form may be used to submit concerns about students' emotional or physical well-being. When completing this form, please be as thorough, specific, and objective as possible about the situation you are reporting, so that we can best determine an appropriate response.
The NCSSM Incident Report form may be used to submit any concerns related to the NCSSM campus, our students, or our staff. When completing this form, please be as thorough, specific, and objective as possible about the situation you are reporting, so that we can best determine an appropriate response.
Students use this form if they are will be missing a curfew check, spending the night off campus, or going at least 30 miles from campus. After the form is submitted, Community Coordinators will call parent/guardians to confirm travel form. Students still need to sign out at a desk before leaving campus.

Maintenance Repair Status: (Must use NCSSM credentials to view)
See when the repair has been officially submitted to maintenance and when maintenance sends confirmation the repair has been completed.
